Laid off employees may file for
unemployment benefits by telephone at
1-877-0HIOJOB (644-6562, Monday through
Friday, 8:00 a.m. to 5:00 p.m., or 24 hours
online at
http://unemployment.ohio.gov.
Please Note: For those employees who
are being laid-off and are considering
drawing out PERS and/or Deferred
Compensation Funds: Please be sure to check
with your One Stop Unemployment Section
first to see how this may impact your
unemployment benefits.
Displaced employees will receive a COBRA Eligibility Notification Form. This is not an enrollment form, but a notification regarding your eligibility to enroll should you elect to continue your Health, Dental, and Vision Insurance coverage through the State of Ohio. This form should be completed whether or not you choose to continue coverage and submitted to your Personnel/Payroll Office no later than 30 days after your last day worked. Upon receipt of a completed and signed eligibility form, an enrollment packet will be sent to you. Please be advised, if you do not complete and submit this notification form indicating your preference to enroll in COBRA, and properly complete and submit enrollment materials timely, your Health, Dental, and Vision Insurance coverage will be terminated at the end of the month following the month in which you are separated. For example, an employee separated June 1, 2011, who chose not to enroll in COBRA would have his/her coverage terminated July 31, 2011. If you wish to continue your Life Insurance for both Group (Basic) and/or Supplemental Life Insurance you will need to complete a Group Life Insurance Continuation (ADM 4302) form and return it to your Personnel/Payroll Office prior to your last day worked. In addition, you should contact the appropriate company as listed below:
