ACT Programs - CFDA #N/A:

Assertive Community Treatment (ACT) teams are a service delivery model for mental health consumers who have extensive needs and have difficulty maneuvering the traditional mental health services system.  A team of professionals (psychiatrists, nurse, substance abuse counselor, vocational specialist, and others) are the single point of clinical responsibility for the client.  Clients are rarely referred to services outside of the team and the clients have 24/7 access to team members.  The team uses a “shared caseload” approach to treatment providing services when and where needs arise.  The ACT teams treat a clearly defined population, are linked to the criminal justice system, and have clear goals for the team members.  The goal of the ACT teams is to engage and retain clients, decrease hospital service usage and increase housing and community stabilization.
